Sussex Partnership NHS Foundation Trust (SPFT)
Pathfinder clinical service provided by nurses and occupational therapists.
The clinical service offers short term input to support people in transition from Sussex Partnership teams and link them to the wider Pathfinder alliance services and other local support as required. They also provide accessible preventative support for people engaged with Pathfinder services with links to Sussex Partnership teams if needed.
Clinicians offer short term individual and/or group work sessions. Groups are co-produced and delivered by Pathfinder clinical staff, Peer Support Workers and Pathfinder partners.
Referrals for the transition service are via SPFT teams within assessment and treatment services only. Referrals for the preventative service are made via staff members within Pathfinder voluntary sector services.

Our advocacy services take action to help you say what you want to say, secure your rights, make your views and wishes heard, and support you to obtain the services that you need. IMHA services are available to those who have been sectioned under the Mental Health Act.
